Responsibilities

As a Home Health Physical Therapist, you will: Provide skilled physical therapy services to adult and geriatric patients in their homes Complete comprehensive physical therapy evaluations, including assessments of: Joint mobility and range of motion Muscle strength and endurance Balance, gait, and functional mobility Transfers and ambulation Home safety and fall risk Develop and implement individualized treatment plans designed to maximize function, mobility, and independence Provide therapeutic exercise, manual therapy, gait training, balance training, and other evidence-based interventions Implement appropriate interventions to promote skin integrity and support wound prevention and healing Collaborate with nursing and the interdisciplinary care team to coordinate patient care Educate patients and caregivers on home exercise programs, fall prevention, safe mobility techniques, and equipment use Reassess patient progress and modify treatment plans as appropriate Complete accurate and timely documentation using an electronic medical record (EMR) system

Qualifications

Current Physical Therapist (PT) license in the State of New Jersey Minimum of one year of recent Physical Therapist experience Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or equivalent accrediting body Home Health experience preferred, but not required Strong clinical assessment, communication, organizational, and critical thinking skills Comfortable using electronic medical records (EMR) Ability to work independently while collaborating with an interdisciplinary care team Reliable transportation and willingness to travel throughout Monmouth, Somerset, and Union Counties Additional Information Base Pay, depending upon
