What You’Ll Do

Provide direct patient care in the home under the supervision of a Registered Nurse Execute individualized care plans and support positive patient outcomes Educate patients and caregivers on disease management and self-care Document care accurately and timely in the EMR Collaborate with physicians and interdisciplinary teams to ensure coordinated care Build trusted relationships with patients, families, and care teams What You Bring Active LPN/LVN license in the state of practice Current CPR certification Minimum of one (1) year nursing experience (home health, community, or med-surg a plus) Strong clinical judgment, communication, and organization skills A patient-first mindset and passion for meaningful care Valid driver’s license, reliable transportation, and auto insurance
