Responsibilities
include (but are not limited to): Coordinate and oversee all direct and indirect patient services provided by home health personnel. Provide guidance and counseling to home health managers to assist them in continually improving all aspects of home health services provided through agency personnel. Collaborates with colleagues to support patients’ transitions to other service lines.
Implement EPIC upgrades. Maintains TJC accreditation. Works with Quality Assurance on CAHPS scores Education/Experience : Degree as a registered nurse.
Bachelor’s degree in a health-related field; BSN preferred. Minimum of three years supervisory experience in a health care (preferably Home Health) setting. Certificates, Licenses, Registrations: Current license as a registered professional nurse in the State of California Home Care Manager Certification, Home Care Administrator Certification preferred.
