Social Worker-Admissions (Hospital Based) (Full Time 0.8/Days)

8 or 64 hrs. per biweekly pay period Days Every 5th-6th weekend, 1-2 holidays per year Essential Functions: Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care, complete comprehensive assessments, and evaluate ongoing interventions. Collaborates with patients, families, and case managers during hospital discharge planning to ensure safe and appropriate transitions of care.

, patients living alone, lacking caregivers, or experiencing mental health/substance use concerns). Performs informational and follow up visits for patients in hospitals, facilities, and home settings. Supports initial response for urgent referrals needing immediate response.

Qualifications

: MSW required LSW or LCSW is also required or must be obtained within 2 years of hire date. Two years’ experience in the health care setting preferred.
