About The Role

: The Crisis Navigator “CN” provides clinical and support services to consumers and/or their families through integrated behavioral healthcare services and resource navigation. The Crisis Navigator “CN” will assist the individual to explore and develop social, occupational, leisure, and living skills, as well as linking to ongoing treatment in the continuum of care. The Resource and Walk-in Crisis Center provides 24/7 support to individuals who are experiencing a mental health crisis, or, looking for resources to prevent a crisis.

This position collaborates with emergency services staff to provide referral and transition services to consumers needing a higher or different level of care. We are seeking professional and compassionate individuals to support clients in crisis and to provide and/or arrange for therapeutic interventions as appropriate for consumers in a crisis/emergency condition. Essential Functions: Provide prevention/early intervention, care navigation, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position.

Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider and family members as needed. Provide brief interventions using evidence-based techniques such as Motivational Interviewing, de-escalation, or other treatments appropriate to support consumer engagement. Document initial intervention, track patient follow-up, and clinical outcomes using an Electronic Health Record system (EHR).

Document in-person and telephone encounters in the EHR, and use the system to identify and re-engage patients. Attend collaborative treatment team meetings and reviews of consumers Provide therapeutic crisis intervention and emergency services as needed Assist eligible consumers in gaining access to needed medical, social, educational, and other services. Provide an assessment of needs, service plan development, referrals/linkage, and monitoring/followups for consumers.
