About The Role

: The Case Manager is an integral member of Community Reach Center’s Outpatient, Community programs, and Case Management Teams. The Care Manager role will emphasize assisting persons to explore and develop social, occupational, leisure, and living skills. Provides services that supports consumers in living in the community successfully including linking consumers with resources to obtain and/or learn basic needs.

Essential Functions: Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families Effective and timely communication across multi-disciplinary teams including external partners Accountability to expectations held by external partners not just those of Community Reach Center Provides outpatient restoration education services for consumers going through competency court proceedings.

Qualifications

: Requires bachelor’s degree Human Services or related field Previous case management experience strongly preferred Previous Experience in court and legal proceedings preferred Communication, organization, time management and clinical skills. Ability to work independently in a community-based setting. Valid Colorado Driver’s license with a clean driving record.

: Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and keyboard.
