Job Title: Social Worker - Learning Disabilities Location: Cardiff Pay Rate: £35 Liquid Personnel is recruiting an Adult Social Worker to join its client’s Learning Disabilities team, based in Cardiff. Supporting adults with learning disabilities to live fulfilling, safe, and independent lives.

Responsibilities

Develop and review outcome‑focused care and support plans that reflect the individual’s strengths, aspirations, and personal outcomes.

Work collaboratively with multidisciplinary professionals including health colleagues, education providers, housing teams, and voluntary sector organisations. Support safeguarding processes by identifying, reporting, and responding to concerns in line with Wales Safeguarding Procedures. Undertake Mental Capacity Act assessments and contribute to best‑interest decision‑making where required.

Provide professional advice and advocacy to ensure individuals’ rights, wishes, and wellbeing remain central to all planning and intervention. Coordinate transitions for young people moving from children’s services into adult learning disability services. Maintain accurate, timely case records and produce high‑quality reports in line with organisational and legislative standards.

Contribute to service development, quality assurance, and continuous improvement initiatives within the Learning Disabilities service.
