Plan, organize and direct home health care services, from initial evaluation and assessment, pain and medication management, and care plan development. Ensure high quality patient experience and positive health outcomes through a comprehensive and personalized plan of care, proactively making revisions based on patient needs.
Prepare complete and timely clinical notes, coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA), and collaborate with the patient’s primary care physician to ensure continuity of care.

Minimum Qualifications
as a Registered Nurse Case Manager (RN CM): Current CA Registered Nurse (RN) license Current CPR card for Healthcare Providers 1 year of Registered Nurse (RN) experience in Home Healthcare, preferred Must be a licensed driver with an automobile that is insured in accordance with state/or organization
Requirements
